Information

This form is to be used when changing a membership type that is paid for by direct debit. If you are moving to a concessionary, corporate or armed forces membership you will be required to show proof of eligibility when you next visit the centre. Without this your change of membership will not be completed

Adjustments to the member’s monthly payments will be made automatically from the next planned direct debit collection date and will be confirmed in writing directly to the member.
