I have read and understood the Health Commitment Statement and agree to participate at my own risk. I am also signing to say that I have waivered the gym induction and will view the induction films online. If you would like or require an induction before starting, please state this when in contact with the leisure centre, ideally before you visit so we can arrange for this to fit in with your paperwork *
I agree to receive text messages and/or emails from Gedling Borough Council in relation to their leisure memberships. *

We will use the information provided by you for providing access to our leisure centres. The basis under which the Council uses personal data for this purpose is that it is necessary for the performance of a contract between you and the Council.
